The short answer: yes. California pays family caregivers through a program called IHSS — In-Home Supportive Services. It is the largest program of its kind in the United States, serving hundreds of thousands of households across all 58 counties. Adult children caring for a parent, parents caring for a child with a disability, spouses, siblings, and even friends can all be paid as IHSS providers — if the person receiving care qualifies.

This guide explains who qualifies, how much IHSS pays in 2026, how taxes work when you live with the person you care for, and exactly how to apply.

What is IHSS?

In-Home Supportive Services is a state program, administered by each county, that pays for personal care so that seniors and people with disabilities can remain safely at home instead of moving to a nursing facility. The person receiving care (the "recipient") is granted a monthly number of care hours, and they choose who provides that care — and in most cases, they can choose you, their family member.

You become the paid "provider": you enroll with the county, log your hours electronically, and receive a paycheck from the state at your county's hourly rate.

Who qualifies?

Two people have to qualify: the person receiving care, and you as the provider.

The person you care for must:

Live in California and live at home (not in a facility).
Be enrolled in Medi-Cal. This is the core requirement — IHSS is a Medi-Cal benefit. If they aren't enrolled yet, that's usually the first step (check status at BenefitsCal.com or call Medi-Cal at 1-800-541-5555). Income rules apply and have changed several times recently, so don't assume you don't qualify based on old information.
Need help with daily living — things like bathing, dressing, meal preparation, medication reminders, or supervision for safety. Age alone isn't the test; the county assesses what help the person actually needs. Recipients can be any age, from young children to seniors.

You, as the provider, must:

Complete the county's provider enrollment (a form, an in-person orientation, and a fingerprint background check via LiveScan).
Be chosen by the recipient as their provider.

There is no requirement to be a certified caregiver or nurse, and you don't need prior professional experience. Adult children, spouses, and — since a rule change in February 2024 — parents of minor children can serve as paid providers with fewer restrictions than in the past.

How much does IHSS pay in 2026?

Each county sets its own hourly wage. As of June 1, 2026, rates range from $16.90/hour to $23.00/hour depending on the county (source: CDSS). A few examples of what that means in practice:

20 hrs/week at a $20/hr county ≈ $1,730/month.
40 hrs/week at a $20/hr county ≈ $3,460/month.

The number of hours isn't up to you — a county social worker assesses the recipient's needs during a home visit and authorizes a monthly number of hours. Authorized hours are capped at 283 per month for recipients classified as severely impaired (less for others). Many family caregivers are authorized for somewhere between 20 and 40 hours per week, but every case is individual.

The tax rule that surprises everyone (in a good way)

If you live in the same home as the person you care for, your IHSS wages are excluded from federal and state income tax under IRS Notice 2014-7 . You claim this by filing a one-time form called the SOC 2298 (Live-In Provider Self-Certification).

Since the 2024 tax season, you still receive a W-2 and still report the income — it simply remains non-taxable.
This exclusion applies to income tax; other payroll rules may still apply. Tax situations vary, so confirm the details with a tax professional for your specific case.

For live-in family caregivers, this rule can effectively add thousands of dollars a year compared to equivalent taxable work.

How to apply: the path from zero to first paycheck

Confirm Medi-Cal. If the person you care for isn't enrolled, apply first — IHSS can't start without it.
Submit the IHSS application (form SOC 295). Submit to the county IHSS office where the recipient lives.
Health care certification (form SOC 873). A licensed health professional certifies the need for care. This form generally must be returned within 45 days of the application.
The home visit. A county social worker visits, assesses needs across daily activities, and authorizes monthly hours. This usually happens within a few weeks of applying.
Provider enrollment. You complete the provider forms (SOC 426 and related), attend a provider orientation, and do LiveScan fingerprinting.
Live-in tax form (SOC 2298). If you live with the recipient, file this to claim the tax exclusion.
Payroll setup. Direct deposit, tax forms, and an Electronic Services Portal (ESP) account for submitting timesheets.
First paycheck. Typically within a couple of weeks after your first submitted timesheet is approved.

From first application to first paycheck, most families should expect the process to take one to three months, depending on the county and on whether Medi-Cal is already in place.

Common questions

Can I be paid to care for my mom or dad? Yes — adult children are among the most common IHSS providers. Your parent must qualify for Medi-Cal and be assessed as needing care.

Can I be paid to care for my child? Yes. Parents of children with disabilities can be paid providers, and restrictions on parent-providers were significantly reduced in February 2024.

Does the recipient's home or savings disqualify them? Not necessarily. Medi-Cal's financial rules have changed substantially in recent years, including major changes to asset rules. Many families who assumed they wouldn't qualify actually do — check current rules rather than relying on what was true a few years ago.

What if the person already receives IHSS from someone else? Recipients can change providers or add providers. If you're taking over care from an agency or another family member, the enrollment process for you as the new provider is the same.

Is IHSS the same everywhere in California? The program rules are statewide, but wages, office procedures, and processing times vary by county.
